29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  Warehouse: Employees were changing the liquefied petroleum fuel (LFP) tank on the Hyster 50 forklift. The employer's PPE hazard assessment did not identify all the hazards associated with changing out the LPF tank on the forklift including frostbite and/or freeze burns.

29 CFR 1910.132(f)(1)(i): The employer did not provide training to each employee who is required by this section to use PPE on knowing when PPE is necessary:  Warehouse: Employees were changing the liquefied petroleum fuel (LPF) tank on the Hyster 50 forklift. Training on the use of hand protection was not provided to employees that changed the LPF tanks. Employees were exposed to frostbite and/or freeze burns.
