29 CFR  1910.151(c):  Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.  Reference: ANSI Z358.1 Emergency Eyewash and Shower Standard 5.4.4 The eyewash unit shall be located on the same level as the hazard and the path of travel shall be free of obstructions that may inhibit the immediate use of the equipment.  In the following locations where corrosive chemical materials were handled, eyewash stations were obstructed and not available for immediate use:  a) Inside Casino, Main Kitchen: The emergency eyewash station was blocked by mops and cleaning buckets.  b) Inside Casino, outside of Burgers & Shakes: The emergency eyewash station was blocked by restroom cleaning stanchions.  c) Inside Casino hallway next to scissors lifts: The emergency eyewash station was blocked with cleaning rags and hand sanitizer station.  Employees were exposed to severe eye burns or blindness.

29 CFR  1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2):    Maintenance Shop:  Employees conducted carpentry tasks such as grinding, drilling, and mechanical cutting that generated wood and metal dusts and were required to wear an 3M N95 respirator.  The employer did not verify that the required workplace hazard assessment had been completed with a written certificate.  The employees were exposed to hazards that could exacerbate underlying medical conditions or create upper respiratory irritation.

The employer did not establish and maintain a respiratory protection program.  The employees were exposed to hazards that could exacerbate underlying medical conditions or create upper respiratory irritation.
