Employees were operating the forklift to transport casino supplies and consumer goods. This condition exposed employees to struck-by hazards.

The plugs were pulled away from the insulation exposing the inner insulated wires. a) Prime Cut Kitchen - The commercial grade Curtis coffee maker plug was pulled away from the insulation exposing the inner insulated wires . b) Loft 94 Kitchen - The commercial grade Curtis coffee maker plug was pulled away from the insulation exposing the inner insulated wires. Employees were exposed to electrical shock and/or fire hazards.
