29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): Poplar Place Plant, Extrusion Dept., Automatic Saw, Line #1. On or about October 22, 2018, the guard on the automatic saw was not adequate to prevent employees from contacting the point of operation (moving saw blade) with their hands/fingers. Employees were exposed to amputation and laceration injuries. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
