29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   Worksite located at 385-399 194th Terrace, Smithville, Missouri- Employees were observed working and walking the tops of the exterior walls on the 2nd level and were not protected from falling by the use of any means of fall protection.

29 CFR 1926.1053(a)(8): A metal spreader or locking device was not provided on each stepladder to hold the front and back sections in an open position when the ladder was being used:   Worksite located at 385-399 194th Terrace, Smithville, Missouri- Employees were exposed to fall hazards in that the employees were using step ladders that were leaned against the walls and did not have their metal spreaders engaged to access the top of the exterior walls of the building.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Worksite located at 385-399 194th Terrace, Smithville, Missouri- Employees were exposed to fall hazards in that employees were accessing the tops of the exterior walls using ladders that did not extend at least 3' above the upper landing surface.
