29 CFR 1926.20(b)(2):   The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):   a)  Building 4, Lee Drive and Building 18, Aldrich Court, Morristown, NJ.  The employer had not provided for frequent and regular inspections of the job site by a competent person for hazards which could impact employees such as fall issues when installing roof shingles in new home construction.                                      Violation observed on or about 03/23/2019.

29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents:  Instance a)  Building 4, Lee Drive and Building 18, Aldrich Court, Morristown, NJ.  Employees using coil nailers were exposed to penetrating eye trauma as no eye or face protection was used.                                      Violation observed on or about 03/23/2019   JA General Construction, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a), which was contained in OSHA inspection number 1129364, citation number 1, item number 1, and was affirmed as a final order on 07/13/2016, with respect to a workplace located at 11-19 Morris Lane, Cranbury, NJ; and also in inspection number 1331869, citation number 1, item number 1, and was affirmed as a final order on 03/12/2019, with respect to a workplace located at Building 48, Nast Road, Morristown, NJ.

29 CFR 1926.501(b)(13):   Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Instance a)  Building 4, Lee Drive and Building 18, Aldrich Court,  Morristown, NJ.  Employees were exposed to falls of approximately 18' on the 4 in 12 and 7 in 12 sloped roofs to the ground below as no methods of fall protection were being used.                                      Violation observed on or about 03/23/2019.    JA General Construction, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1331869, citation number 1, item number 1, and was affirmed as a final order on 03/12/2019, with respect to a workplace located at Building 48, Nast Road Morristown, NJ; and also in inspection number 1290478, citation number 1, item number 1, and was affirmed as a final order on 06/13/2018, with respect to a workplace located at 113 Columbia Road, Building #44, at the Residences @ Columbia Park, Morristown, NJ.

29 CFR 1926.502(d)(16)(iii):   Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  Instance a)  Building 4, Lee Drive, Morristown, NJ.  Employees applying roofing material were exposed to a free fall of more than 6 feet (~18 feet to the ground below) from the 4 in 12 sloped roof as their rope lifelines were not anchored to anything and were just lying on the roof.                                            Violation observed on or about 03/23/2019.  .  JA General Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.502(d)(16(iii), which was contained in OSHA inspection number 1290478, citation number 1, item number 2, and was affirmed as a final order on 06/13/2018, with respect to a workplace located at 113 Columbia Road, Building 44, Morristown, NJ.
