29 CFR 1926.150(a)(2): Access to all available firefighting equipment was not maintained at all times: Worksite located at 2300 Independence Ave., Kansas City, Missouri- Employees were exposed to fire hazards and were not provided any means to extinguish a fire while on the scaffold. A portable gasoline container lwas located on the scaffold in which it was used to refuel the masonry saw and employees.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: Worksite located at 2300 Independence Ave., Kansas City, Missouri- Employees were exposed to fall hazards while working on a fabricated framed scaffold in which the scaffolding which was missing multiple guardrails on multiple sections of the scaffold. The Decal Construction, dba Forbes & Kerissler, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(4)(I), which was contained in OSHA inspection number 1174798, citation number 1, item number 1and was affirmed as a final order on November 21, 2016, with respect to a workplace located at 3410 Market Place, Lee's Summit, Mo 64082.
