29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  a)    On or about 8/20/2019, Employee entered gated conveyor area for Metl saw without locking out machine which she had not received specific lockout instruction training for creating a struck by hazard.

29 CFR 1910.147(d):  The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a)    On or about 8/20/2019, an employee entered the gated area of the Metl Saw, and did not follow the required steps of Lockout prior to engaging in the servicing and/or maintenance activity.  As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a)    On or about 8/20/2019, Employees handling PPG SPECTRACON GXA ACID CATALYST and PPG CLEAR ACRYLIC TOPCOAT in the paint kitchens did not have suitable facilities for quick drenching or flushing of the eyes and body readily available for emergency use. Hazards listed on the SDS for both products included serious eye damage and severe skin burns for the ACID CATALYST.  b)    On or about 8/21/2019, Employee filling floor sweeper in Building #6 with NUBLEND Dual Draw 1 A HD alkaline Base and DUAL DRAW 1B PE SURFACTANT BLEND did not have suitable facilities for quick drenching or flushing of the eyes and body readily available for emergency use. Hazards listed on the SDS for both products included severe skin burns and eye damage.

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:  a)    On or about 8/20/2019, Point of operation guards on Metl Saw were not adequate to prevent contact with sawblade creating an amputation hazard. Outfeed hinged guard was not secured or interlocked.
