29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a) Crown Battery Manufacturing Co. located at 1445 Majestic Drive in Fremont, Ohio; On or about March 16, 2020 the employer did not ensure that that employees were properly trained in the lock out tag out procedures and associated energy sources while clearing jammed battery plates in COS line 2 folder. Employees were exposed to amputation and caught in hazards in that the energy sources were not isolated prior to engaging in this activity.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Crown Battery Manufacturing Co. located at 1445 Majestic Drive in Fremont, Ohio; On or about March 16, 2020 the employer did not ensure that employees utilized energy control procedures prior to clearing a jammed plate in the folder of COS Line 2. Employees were exposed to amputation and caught in hazards in that the energy sources were not isolated prior to engaging in this activity. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)], are not implemented to protect employees from machine servicing hazards such as but not limited to, clearing jammed plates.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Crown Battery Manufacturing Co. located at 1445 Majestic Drive in Fremont, Ohio; On or about March 16, 2020 the employer did not ensure the in-going nips points located on the COS Line 2 were guarded. Employees were exposed to amputation hazards in that the in going nip point created between the two slyver rollers was unguarded.
