Casino Bar: On or before June 22, 2021, the employer did not ensure the Refrigerated Glass Door Merchandiser and blender appliance were plugged directly into the wall electrical outlet and were in use in accordance with listed instructions. The fridge and blender were plugged into a power strip which was plugged into a temporary extension cord. Employees were exposed to potential electrical shock and /or fire hazard(s).

29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage:  Work Area Between Office Trailers: On or before June 22, 2021 the employer did not ensure temporary power extension cord(s) were protected from accidental damage. An extension cord was run to the employee break area, which powered a Hessaire Cooling Solutions (Swamp cooler). Heavy metal chair bases were placed on top of the extension cord, which caused the electrical cord to be pinched. This condition exposed employees to the potential of electrical shock hazard(s).

29 CFR  1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:   Casino Maintenance Connex Shop: On or before June 22, 2021 the interior right side of the shop electrical panel, the employer used a yellow/black flexible extension cord with the ends removed with one end of the cord tied directly into the electrical panel and the other end was tied into an electrical receptacle (outlet box). Employee(s) had used this extension cord to power tools and equipment such as a table saw. This condition exposed employees to potential electrical shock hazard(s).

29 CFR  1910.334(a)(2)(i):Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  Casino Maintenance Connex Shop: On or before June 22, 2021 the interior right side of the shop, the yellow/black extension cord was not inspected before use and was tied directly into the electrical panel.  The extension cord had damage to the outer jacket insulation exposing the inner wires.  Employees had used this extension cord to power tools and equipment such as a table saw. The blower plug was pulled away from the insulation exposing the inner insulated wires. Employees were exposed to electrical shock hazard(s).  The Red Earth Casino was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(g)(2)(iii), which was contained in OSHA inspection number_1385395_, citation number_1_, item number_6_ and was affirmed as a final order on May 21, 2019 with respect to a workplace located at Red Earth Casino 3089 Norm Niver Road THERMAL, CA 92274
