29 CFR 1926.451(g)(1):Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. (a) On or about June 24, 2021, an employee conducting carpentry operations from a supported scaffold ( working platform elevated by a rough terrain forklift) was not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 11 feet.

29 CFR 1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) On or about June 23, 2021, employees installing windows from a steep pitch overhang (south elevation), were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 12 feet. Finishing Edge Carpentry LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.501(b)(13)), which was contained in OSHA inspection number 1315719, citation number 1, item number 2a and was affirmed as a final order on June 28, 2018, with respect to a workplace located at 8740 S. Avain Way, Franklin, WI 53132.
