29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) On or about March 7, 2024, at  a jobsite  located at 456 Kiddle Ln., Union Grove WI. the portable extension ladder being used to access the upper landing area or roof of the structure did not extend at least three feet above the point of contact and was not secured. This exposed employees accessing and egressing to and from the roof to a fall hazard of approximately 12 feet, 2 inches.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a) On or about March 7, 2024, at  a jobsite  located at 456 Kiddle Ln., Union Grove WI, employees conducting framing work on a 6:12 pitch roof of the residential new construction home were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 12 feet, 2 inches, on the East side of the structure.  Finishing Edge Carpentry, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) , which was contained in OSHA inspection number 1584205, citation number 1, item number1, and was affirmed as a final order on August 8, 2022, with respect to a workplace located at 9749 Bergamont Dr., Franklin, WI 53132.  Finishing Edge Carpentry, LLC.  was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1524945 citation number 2, item number 1, and was affirmed as a final order date of October 27, 2021, with respect to a workplace located at 8036 98th Circle Pleasant Prairie WI 53158.
