29 CFR  1910.178(a)(6): The employer did not ensure that all nameplates and markings were in place:  Facilities Maintenance Yard: Employees operated the Hyster Forklift(s) to move digital signs .  The Green Hyster A2 Forklift did not have the  instruction data nameplates  to indicate load limits of the forklift. This condition exposed the employees to forklift overloads which could cause forklifts to overturn and/or tip over hazard(s).

The outlet was tied into and extension cord cable which was used as fixed wiring.  Employees were exposed to electrical and/or fire hazards.

29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  In the follow locations, appliances were plugged into power taps and/or extension cords and were not used according to listing and labeling instructions.   a) Facilities Maintenance Trailer Shelf- Black Mini-refrigerator plugged into an orange extension cord. b) Main Kitchen Prep Line- The food warmer and Coke Machine. c) Bar - Fridge was daisy chained with a power tap and extension cord. d) Travel Center- Vendor Fridge(s) daisy chained with a power tap and extension cord    Employees were exposed to potential risk of fire and/or electrical shock.  The Golden Acorn Casino and Travel Center was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 1369787, citation number 1, item number 4 and was affirmed as a final order on January 31, 2019 with respect to a workplace located at 1800 Golden Acorn Way CAMPO, CA. 91906
