29 CFR  1910.151(c):  Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.  Reference: ANSI Z358.1 Emergency Eyewash and Shower Standard 5.4.4 The eyewash unit shall be located on the same level as the hazard and the path of travel shall be free of obstructions that may inhibit the immediate use of the equipment.  In the following locations where corrosive chemical materials were handled, eyewash stations were obstructed and not available for immediate use:  a) Inside Casino, Maintenance Shop: The emergency eyewash station was blocked by brooms, cans, and equipment.   b) Inside Casino, Inside Cylinder/Boiler Room: The emergency eyewash station was blocked by floor cleaning equipment, mop buckets, and brooms. Employees were exposed to eye burns or blindness.

The employer did not verify that the required workplace hazard assessment had been completed with a written certificate.  The employees were exposed to skin,  eye irritation, and hazards that could exacerbate underlying medical conditions or create upper respiratory irritation.
