29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. On or about 8/17/2021, employees from Bastin Roofing, LLC were installing roofing materials on a roof of a residential building located at 110 Tuscany Lane in O Fallon, MO. The foreman and two employees were using an extension ladder to access the roof. The ladder was not extended three feet beyond the landing surface or edge of the roof.

On or about 8/17/2021, employees from Bastin Roofing, LLC were installing roofing materials on a roof of a residential building located at 110 Tuscany Lane in O Fallon, MO. The foreman and two employees were observed working on the roof. They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems. Bastin Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1379803, citation number 1, item number 1, issued on 2/27/2019, and was affirmed as a final order on 3/21/2019 with respect to a workplace located at 65 Snowberry Ridge Court, O Fallon, MO 63366.
