On or about 8/17/2021, employees from Bastin Roofing, LLC were installing roofing materials on a roof of a residential building located at 110 Tuscany Lane in O Fallon, MO.  The foreman and two employees were using an extension ladder to access the roof.  The ladder was not extended three feet beyond the landing surface or edge of the roof.
