29 CFR  1910.157(c)(1): A portable fire extinguisher was blocked and not available for immediate use by employees.  Northeast Mechanical Tool Shed:  Where the employer provided portable fire extinguishers, the extinguisher on the South side of the shed entryway was blocked from access.  This condition exposed the employees to hazards associated with fire such as burns and smoke inhalation.

29 CFR  1910.178(q)(1):  Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:  North Course Exterior Shop Area: The LA 1001 Kubota Front loader was found with inoperable break lights and no back up alarm and was not removed from service.  Employees were operating the Kubota to transport green waste trash into storage stall.  This condition exposed employees to no advanced warning such as backing up, stopping and/or struck by hazard(s).

North Course Exterior Shop Area: The employer did not examine the LA 1001 Kubota Front loader for defects that would impair the safe operation daily. The loader was found with inoperable break lights and no back up alarm. Employees were operating the Kubota to transport green waste trash into storage stall. This condition exposed employees to no advanced warning such as backing up, stopping and/or struck by hazard(s).

29 CFR  1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  In the follow locations, appliances were plugged into power taps and were not used according to listing and labeling instructions.  a) Maintenance Shop, West Superintendents office - The refrigerator was plugged directly into a relocatable power tap. b) Administration Building Breakroom- The microwave, toaster and coffee maker were plugged directly into a relocatable power tap.     Employees were exposed to electrical shock and/or fire hazards.

29 CFR  1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  Outside Exterior of Maintenance Shop: The orange 3 way adapter being used to power the grinder and fan had not been visually inspected before use for defects and was found with a broken ground pin in it.   Employee(s) had used the grinder to grind worn mower reels and the fan was used for air circulation.  Employee(s) were exposed to electrical and/or fire hazard(s).
