29 CFR  1910.106(d)(2)(i):  Approved containers and portable tanks were not used to store flammable or combustible liquids.  Maintenance shop:  Landscaping:  An employee in the maintenance department dispensed, transported, and stored a category 1 flammable liquid, gasoline, in plastic containers.  The employer had not used approved containers such as metal safety cans to hold category 1 flammable liquids.  The employee was exposed to fire hazards.

29 CFR  1910.151(c):  Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  Golf Course:  Kitchen:  An employee used corrosive chemicals such as Oasis 146 Multi-Quat sanitizer, Ecolab Sanitizing Wash-N-Walk, Ecolab Greasecutter Plus, and Ecolab Lime-a-way to conduct dish, pots and pan washing, floor, and equipment cleaning operations without access to suitable facilities for quick drenching or flushing of the eyes or body within the work area for immediate emergency use.   The employee was exposed to eye damage and burns.

29 CFR  1910.132(f)(2):The employer did not require employees to demonstrate an understanding of the training specified in paragraph (f)(1) of this section, and the ability to use PPE properly, before being allowed to perform work requiring the use of PPE:  South Course:  Kitchen:  An employee used corrosive chemicals such as Oasis 146 Multi-Quat sanitizer, Ecolab Sanitizing Wash-N-Walk, Ecolab Greasecutter Plus, and Ecolab Lime-a-way to conduct dish, pots and pan washing, floor, and equipment cleaning operations without using safety goggles.  The employer did not ensure the employee understood specific hazards of the chemicals such as eye protection used prior to the employees performing work.  The employee was exposed to eye damage and burns.

The employer did not conduct a hazard analysis specific for the health concerns of the chemicals to include an evaluation if respiratory protection personal protective equipment or eye protection was required.   Employees were exposed to potential carcinogens, eye damage, and respiratory irritation.
