29 CFR 1910.119(e)(3)(i): The process hazard analysis did not address the hazards of the process: Toledo Refining Company LLC � Oregon, Ohio: On or about January 6, 2022, the employer did not ensure that the most recent process hazard analysis (PHA) sufficiently evaluated the safe relief of flammable hydrogen gas to atmospheric discharge from the Unit 9-2 Hydrocracker as it did not document review of the prior dispersion modeling and determine whether any changes had been made that warranted a re-evaluation of the safety of the discharge point.
