29 CFR 1910.119(e)(3)(vi):The process hazard analysis did not address human factors. Toledo Refining Company, LLC located at 1819 Woodville Road in Oregon, Ohio 43616: On or about June 28, 2023 the employer did not address human factors in the process hazard analysis when loading railcars by means of a manual single block valve. Employees were exposed to hazards associated with toluene such as but not limited to skin irritation, dizziness, organ damage, and potential for fire and explosion in that the single block quarter turn valve located on the loading arm of 4-Track, 1-Spot was not closed after filling a rail car and spilled approximately 1,800 gallons of toluene onto the ground.
