1910.178(o)(1)  The employer did not ensure that only stable or safely arranged loads were handled:   a.) Pennex Aluminum Company, LLC; Furnace B - On or about 2/12/22 and at times prior thereto, a natural gas line was not protected against being struck by aluminum logs. A gas line that was attached to Furnace B and positioned next to the lay down table was struck by a large aluminum log when the log was being manipulated by the forklift.  The gas line strike caused a fire when the natural gas was released.  ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
