The employees were exposed to a struck-by overhead hazard. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

1910.147(c)(7)(iii)(A) Retraining was not provided for authorized and affected employees when there was a change in their job assignments, a change in machines, equipment or processes that presented a new hazard, or when there was a change in the energy control procedures (a) 50 Community Street, Wellsville, PA; Press B - On or about February 11, 2026, employees conducting lockout at the main electrical disconnect were exposed to the moving parts of the press or the transverse conveyor. Employees did not have retraining, as appropriate, which outlining how to safely access the disconnect. Employees were exposed to caught in, crush, and struck by hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) 50 Community Street, Wellsville, PA; Press A - On or about February 11, 2026, employees working at or near the press had to reach over the path of movement of the die shuttle to access a control panel and emergency stop. Employees were exposed to a caught in and struck by hazard. (b) 50 Community Street, Wellsville, PA; Press B - On or about February 11, 2026, employees working at or near the press were exposed to the path of movement of the die shuttle when applying wax to or cleaning the press flare. Employees were exposed to a caught in and struck by hazard. (c) 50 Community Street, Wellsville, PA; Press B - On or about February 11, 2026, employees cleaning the flare were exposed to moving parts of the press which were outside of the light curtained guarding. Employees were exposed to a caught in, crush, and struck by hazard. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
