On or about September 5, 2023, Pennex Aluminum Company LLC. at 1 Commerce Avenue Leetonia, OH 44431, did not ensure that employees conducting maintenance in the scrap shear hydraulics area, approximately 3 feet 10 inches above, were protected from falling into or onto the adjacent scraps chopping (SHEAR) machine and conveyor below.

On or about September 5, 2023, Pennex Aluminum Company LLC. at 1 Commerce Avenue Leetonia, OH 44431, did not ensure the height of stair rail system leading down to the Scraps Chopping (SHEAR) Machine and Conveyor for Press D was not less than 42 inches (107 cm) from the leading edge of the stair tread to the top surface of the top rail.
