29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft: In December of 2021, four employees performing maintenance on turbines in the Cogen plant did not each affix a personal lockout device to each source of energy, exposing employees to amputations and crush-by hazards.

29 CFR 1910.215(a)(4):Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: Landscaping Department. Employees use a Jet 8 inch bench grinder model JBG-8A, S/N 14127999 to sharpen lawnmower blades. (a) Landscaping Department. The right hand work rest of the Jet 8 inch grinder model JBG-8A, S/N 14127999 was adjusted so that there was a 1 inch gap between the work rest and the abrasive wheel, exposing employees in the blacksmith area to caught-in and struck-by hazards; (a) Landscaping Department. The left hand work rest of the Jet 8 inch grinder model JBG-8A, S/N 14127999 was adjusted so that there was a 1 and 1/4 inch gap between the work rest and the abrasive wheel, exposing employees in the blacksmith area to caught-in and struck-by hazards.

29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Cogen Plant: In December of 2021, employee(s) were servicing the turbines without a procedure developed, documented, and utilized for controlling all hazardous energy from rotating parts. Employees are exposed to entanglement hazards while performing maintenance work without equipment fully de-energized. Pechanga Resort and Casino was previous cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1453923, citation number 1 item number 2 and was affirmed as a final order on February 13, 2020, with respect to a workplace located at 45000 Pechanga Pkwy, Temecula, CA 92592.
