29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  Maintenance Tool Crib in Facilities Shop: On or about November 6, 2018, and at times prior thereto, at least once per day employees used the Baldor Bench Grinder to sharpen tools, bits, and knives to cut boxes. The adjustable tongue guard was not adjusted to within one-fourth inch of the abrasive wheel as it was one-half inch at time of inspection. Employees were exposed to struck-by-hazards.

29 CFR 1910.101(a): The employer did not determine that compressed gas cylinders under his control were in a safe condition to the extent that can be determined by visual or other inspections conducted as prescribed in the Hazardous Materials Regulations; Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:     Compressed Gas Storage Area: On or about November 6, 2018, and at times prior thereto, employees placed and removed compressed gas cylinders in/from the storage area on a daily basis. Four empty compressed gas cylinders (each weighed approx. 10 lbs) were not secured to prevent from tipping over; in addition, the cylinders did not have valve caps. Employees were exposed to struck-by-hazards.
