29 CFR  1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.   (a)  M/I Homes - Crystal Springs Colony Farms, Zephyrhills (FL): On or about 4/7/2022, the employer exposed employees to fall hazards in that, an employee without fall protection was working near a window cut out area on the second story exposed to a fall to the ground below.

29 CFR  1926.1052(c)(1)(ii):Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:  (a) M/I Homes Crystal Springs Colony Farms:  On or about April 7, 2022, employees were exposed to falls while using two sets of wooden stairs to access second story that were not equipped with a starrail system.
