29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) On or about April 12, 2022, at the above addressed jobsite, an employee using a portable circular saw did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.  a) On or about April 12, 2022, at the above addressed jobsite, employees performing sheething operations on a roof eleven (11) feet and five (5) inches above the ground were not protected from falls, thereby exposing employees to fall hazards.  TBR Construction, LLC was previously cited for a violation of this occupational safety and health standard  29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1353642, citation number 02, item number 01, and was affirmed as a final order on February 4, 2019, with respect to a workplace located at Lot#86 Olde Stone Village Frankfort, IL. 60423.

29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   a) On or about April 12, 2022, at the above addressed jobsite, an employee performing residential carpentry operations from a Genie GTH-644 rough terrain forklift, thirteen (13) feet five (5) inches above the ground , was exposed to injuries from falls due to the lack of guardrails.  TBR Construction, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(g)(1), which was contained in OSHA inspection number 1353642, citation number 01, item number 01, and was affirmed as a final order on February 4, 2019, with respect to a workplace located at Lot#86 Olde Stone Village Frankfort, IL. 60423.
