29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  At the jobsites located at 17733 Slater St. Overland Park, Kansas, employees engaged in residential framing activities were exposed to falls from approximately 20 feet from the roof to the ground below, in that the aluminum ladder used to access the roof did not extend  at least 3 feet above the eave of the roof.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Employees were exposed to fall hazards. At the jobsites located at 17733 Slater St. Overland Park, Kansas,  employees engaged in roofing activities were exposed to a fall distance of approximately 22.5 ft. from the roof to the ground below. Employees were not using fall protection.  The Ovalle Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1323744, citation number 1, item number 2 and was affirmed as a final order on 10-22-2018, with respect to a workplace located at 9819 Brockway in Lenexa, Kansas .
