On or about September 15, 2022, at a worksite in Odessa Florida, employees were using a fiberglass self-supporting (A-frame) ladder to gain access from the first story/ garage rooftop to the second story roof top, wherein the ladder, while in the closed/ stowed configuration, was leaned up against the second story roof eve; therefore exposing the employees to a fall hazard.
