29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards: a) Jobsite located at or near 1755 N Howard St, Philadelphia, PA - On or about September 20, 2022, the employer did not provide a training program for each employee who were exposed to fall hazards. Employees were framing the second story of a residential structure and were not trained on fall hazards, including how to protect themselves from such hazards, thereby exposing employees to falls of approximately 10 feet.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: a) Jobsite located at or near 1755 N Howard St, Philadelphia, PA - On or about September 20, 2022, the employer did not ensure employees engaged in residential construction activities 6 feet or more above lower levels were protected by guardrail systems, safety net system, or personal fall arrest system. Employees were framing the second story of a residential structure without the use of fall protection, thereby exposing employees to fall hazards of approximately 10 feet. Holanda Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1575334, citation number 1, item number 1, and was affirmed as a final order on May 16, 2022 with respect to a workplace located at 689 Stoneyford Road, SOUTHAMPTON, PA 18966.
