Employee was required to perform masonry work at a height of approximately 38 feet from a scaffold platform that was not fully planked, thereby exposing employees to fall hazards.

Employees were required to perform masonry work utilizing a scaffold approximately 38 feet high and 42 inches wide that had not been restrained from tipping, thereby exposing employees to fall and struck-by hazards.

Employees were required to perform masonry work atop a scaffold approximately 38 feet high with the safety lanyard attached to the front of the body harness while improperly tied off to the anchor point, thereby exposing employees to fall hazards up to approximately 38 feet.

Holanda Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1575334, citation number 1, item number 1 and was affirmed as a final order on May 26, 2022, with respect to a workplace located at 689 Stoneyford Road Southampton, PA.
