29 CFR  1926.20(b)(1):The employer did not initiate and maintain an accident prevention program as may be necessary to comply with this part.  a) Jobsite: 1324 Frankford Ave. Parcel B Phila., PA 19125 - On or about January 17, 2023, the employer did not initiate or maintain an accident prevention program to address the associated hazards that employees are exposed to during framing activities, such as but not limited to fall hazards.

29 CFR  1926.503(a)(1):The employer did not train each employee who might be exposed to fall hazards to enable them to recognize fall hazards and the procedures to be followed in order to minimize these hazards:  a) Jobsite: 1324 Frankford Ave Parcel B Phila., PA 19125 - On or about January 17, 2023, The employer did not provide a training program for each employee conducting framing activities on a residential building in the recognition and avoidance of fall hazards.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of this section.  Jobsite: 1324 Frankford Ave. Parcel B Phila., PA 19125 - On or about January 17, 2023, the employer did not ensure that an employee conducting framing operations in the residential construction building were protected from falls by the use of a fall protection system.   a) From the roof of a stair tower approximately 50 feet above ground level.  b) From the 5th level, approximately 40 feet above ground level.  c) From a plank extending from a wall on the 5th level approximately 20 feet above the next lower level.  Holanda Construction, LLC was previously cited for a violation of this occupational safety and health standard which was contained in OSHA Inspection number 1575334, Citation number 1, Item number 1 and was affirmed as a final order on 5/16/22, with respect to a workplace located at 689 Stoneyford Road Southampton, PA 18966.
