29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  A) 1880 Saw Mill River Road, White Plains, - NY:  An employee was exposed to head injuries while working below roofing work activities. The employee was not protected by a hard hat or protective helmet, on or about November 19th, 2022.  Serrano Contracting, Inc was  previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a ), which was contained in OSHA inspection number 1429792, citation number 1, item number 1 and was affirmed as a final order on 10/10/19, with respect to a workplace located at 239 Main Street in Eastchester, NY.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   A) 1880 Saw Mill River Road, White Plains, - NY:  Employees were exposed to fall hazards of approximately 23 feet while engaged in roofing work on a structure. The employees were not protected by any type of fall protection system, on or about November 19th, 2022.  Serrano Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1585914, citation number 1, item number 1 and was affirmed as a final order on 12/12/22, with respect to a workplace located at 130 Orange Avenue in Suffern, NY.

A) 1880 Saw Mill River Road, White Plains, - NY:  Employees were exposed to fall hazards of approximately 8 feet while using a folding step ladder that was not opened and locked into place, on or about November 19th, 2022.  Serrano Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.1053(b)(4), which was contained in OSHA inspection number 1544117, citation number 1, item number 5 and was affirmed as a final order on 4/18/22, with respect to a workplace located at 14 N. Broadway, Tarrytown, NY.
