The extension ladder used only extended approximately 8 inches above the landing; on or about, November 20th, 2024.

Each employee engaged in residential construction activities 6 feet or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest systems: A) 8 Old Jerome Avenue, Yonkers - NY: Employees were exposed to fall hazards of approximately 12 1/2-feet while conducting roofing & siding work while working on a roof without the use of personal fall arrest, or other fall protection systems, on or about, November 20th, 2024. Serrano Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926. 501(b)(13), which was contained in OSHA inspection number 1635136, Citation number 1, Item number 2, and was affirmed as a final order on April 20th, 2023, with respect to a workplace located at 1880 Saw Mill River Road, White Plains, NY.
