29 CFR  1910.157(e)(3): Portable fire extinguishers were not subjected to an annual maintenance check:   Located at the Yreka Service Center, multiple portable fire extinguishers were not subjected to an annual maintenance check:  1. Three portable fire extinguishers were located in or near the road crew office with annual maintenance last completed in 2021. 2. One portable fire extinguisher was located in the genetics building storage area with an annual maintenance last completed in May 2022. 3. One portable fire extinguisher in the fire cache was last maintained in May 2022.   Employees are exposed to fire hazards.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries.  At the Yreka Service Center, five fire extinguishers were not mounted, located, and identified so that they were readily accessible to employees. Employees are exposed to fire hazards. 1). One fire extinguisher was on the floor in the fleet maintenance shop next to an out-of-service Rockwell drill press 2). Two fire extinguishers were on a table in the road crew office 3). One fire extinguisher was on the floor in the genetics building storage area 4). One fire extinguisher was near the road crew office on the floor  The USDA - Forest Service was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.157(c)(1), which was contained in OSHA inspection number 1415093, citation number 1, item number 3 and was affirmed as a final order on 09/09/2019, with respect to a workplace located at 6409 N Air Terminal Rd, Douglas AZ 85607.

29 CFR 1910.253(b)(4)(iii):Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:  At the Yreka Service Center in the fleet maintenance shop and the road crew maintenance shop, there were multiple oxygen cylinders stored less than 20 feet from acetylene fuel-gas cylinders or combustible materials, exposing employees to fire hazards. This was observed in the following areas: 1). In the fleet maintenance shop, one oxygen cylinder next to one acetylene cylinder with regulators attached on a welding cart lacking noncombustible barrier 2). In the fleet maintenance shop, two oxygen cylinders next to one acetylene cylinder, capped 3). In the road crew shop, one oxygen cylinder next to one acetylene cylinder, capped, near the wall and a set of jacks 4). In the road crew shop, one oxygen cylinder next to one acetylene cylinder on a welding cart lacking a noncombustible barrier  The USDA - US Forest Service - Klamath National Forest was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.253(b)(4)(iii), which was contained in OSHA inspection number inspection 1481084, citation number 1, item number 2, and was affirmed as a final order on October 21, 2020, with respect to a workplace located at 538 Oak Knoll Road, Klamath River, CA 96050.

29 CFR  1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:  Located in the fleet maintenance shop, one outlet did not have a faceplate on it, exposing employees to electrical hazards.  The USDA - Forest Service was previously cited for violations of this Occupational Safety and Health Standard 29 CFR 1910.305(b)(2)(i) which are contained in the following OSHA inspection numbers:  1). Inspection 1608178, citation number 1, item number 1, and was affirmed as a final order on 09/01/2022, with respect to a workplace located at 1090 Glendora Mountain Road, Glendora, CA 91741. 2). Inspection 1628769, citation number 2, item number 1, and was affirmed as a final order on 12/21/2022, with respect to a workplace located at 49870 State Hwy 139, Tulelake, CA 96134.

29 CFR 1910.303(g)(1):  Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.  Sufficient access and working space was not provided and maintained about electric equipment as required.  1.) Located in the fleet maintenance shop behind the door, there was insufficient space in front of the electrical panel powering lights and outlets. Items were in front of the panel including a first aid kit, a box, and cones and signs.  2). Located in the road crew maintenance shop, one electrical panel powering lights and outlets had a metal desk protruding into the minimum required working space in front of the panel.   Employees are exposed to electrical and fire hazards.  The U.S.D.A. - Klamath National Forest was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1478849, citation number 2, item number 2 and was affirmed as a final order on 10/21/2020 with respect to a workplace located at 1711 South Main St, Yreka, CA 96097.
