29 CFR 1910.132(d)(1): The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall ensure the completion of the activities identified in sections 1910.132(d)(1)(i)-(iii). On or about June 4, 2023, with regard to an establishment located at 880 South View Drive, Mosinee, WI 54455, the employer did not ensure that employees engaged in activities related to unjamming metal extrusions were protected from potential thermal burns to their face, hands, arms, and torso with Personal Protective Equipment capable of protecting them from the extreme heat of the extruded metal.

29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the items identified in section A-D of this standard. On or about June 4, 2023, with regard to an establishment located at 880 South View Drive, Mosinee, WI 54455, the energy control procedure that was developed by the employer did not account for hazardous thermal, kinetic, and gravitational forces associated with trapped / jammed material while unjamming aluminum extrusions on Press #11.
