29 CFR  1910.157(c)(4): The employer did not assure that portable fire extinguishers  were maintained in a fully charged and operable condition and kept in their designated areas at all times except during use:  At the Ash Mountain maintenance facility in the auto shop, one portable fire extinguisher was not in its designated area in front of a gray cabinet. Portable fire extinguisher markings were present, but the extinguisher was not. Employees were exposed to fire hazards.

29 CFR  1910.212(a)(1): Types of guarding. One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  Located in the automotive shop at Ash Mountain, a red Arcan manual shop press did not have machine guarding, exposing employees to hazards including flying metal objects or contact with hydraulically-powered machine parts. These hazards can lead to crushing injuries, broken bones, or lacerations.   The U.S. Department of the Interior - National Park Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1634386, citation number 5, item number 1 and was affirmed as a final order on 12/29/2022, with respect to a workplace located at 500 Aubell Lane, Crescent City, CA, 95531.

29 CFR  1910.212(b): Machine(s) designed for fixed location(s) were not securely anchored to prevent walking or moving:  1). Located at Ash Mountain in the Trails area, a JET model J-2530 bench drill press was not securely anchored to prevent walking or moving.  2). Located at Ash Mountain in the automotive shop, the red Arcan manual shop press was not securely anchored to prevent walking or moving.   Employees are exposed to machine tipover hazards.  The U.S. Department of Interior - National Park Service was previously cited for violations of this occupational safety and health standard 29 CFR 1910.212(b) which are contained in the following OSHA inspections:  1). OSHA inspection number 1515048 citation number 2, item number 2 and was affirmed as a final order on 05/07/2021 with respect to a workplace located at 83715 Park Road, Kings Canyon, CA 93633 (Sequoia National Park).  2). OSHA inspection number 1515056 citation number 1, item number 4 and was affirmed as a final order on 05/07/2021 with respect to a workplace located at Ash Mountain Maint. Facility, 47050 Generals Highway, Three Rivers, CA 93271.  3). OSHA inspection number 1515054 citation number 2, item number 2 and was affirmed as a final order on 05/07/2021 with respect to a workplace located at Red Fir Facility, 64600 Generals Highway, Sequoia National Park, CA 93262.

29 CFR  1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:  Located at the Ash Mountain Maintenance Facility, there were three instances in which oxygen cylinders in storage were not separated from acetylene fuel-gas cylinders a minimum distance of 20 feet or by a noncombustible barrier. Employees were exposed to fire hazards.   1). In the auto shop in front of an anvil, two welding carts were stored, each with one cylinder of oxygen and one acetylene fuel-gas cylinder stored inches from one another.    2). In the auto shop next to an orange drill press and a peg board with tools on it, six oxygen cylinders were stored next to one acetylene fuel-gas cylinder.   3). In the auto shop next to a gray drill press, one welding cart was stored, with a large oxygen cylinder and a smaller acetylene fuel-gas cylinder inches from each other with the regulators not connected.  The U.S. Department of the Interior - National Park Service was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.253(b)(4)(iii), which was contained in OSHA inspection number 1634386, citation number 3, item number 1 and was affirmed as a final order on 12/29/2022, with respect to a workplace located at 500 Aubell Lane, Crescent City, CA 95531 (Redwood National and State Parks).

29 CFR  1910.1200(f)(6)(ii): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with product identifier and words, pictures, symbols, or combination thereof, which provided at least general information regarding the hazards of the chemicals:  Located in the Ash Mountain Maintenance Facility auto shop near the used oil storage, a large pot on the ground contained an unknown orange chemical with parts soaking in it. The pot did not have a product identifier or information about the hazards of the chemicals.   The U.S. Department of the Interior - National Park Service was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.1200(f)(6)(ii) which was contained in OSHA inspection number 1502671, citation number 1, item number 2, and was affirmed as a final order on 02/19/2021, with respect to a workplace located at P.O. Box 588, Indian Route 7, Chinle, AZ 86503 (Canyon de Chelly National Monument).
