29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: Located at the Wolverton water treatment plant, the employer did not ensure each employee on a walking-working surface with an unprotected edge greater than 4 feet was protected by falling when working over the water tanks during tasks such as changing light bulbs that are mounted on the steel beams.

29 CFR 1910.28(b)(3)(i): Holes. The employer did not ensure each employee was protected from falling through any hole (including skylights) that was 4 feet (1.2 m) or more above a lower lever by one or more of the following: (A) Covers; (B) Guardrail systems; (C) Travel restraint systems; or (D) Personal fall arrest systems. Located at the Wolverton Water Treatment Plant, three concrete water chambers on the lower walkway were not covered to prevent employees from falling into them. The chambers sometimes have water inside them at a depth of greater than 6'. Employees are exposed to open holes and potential falls, which can lead to head injuries or drowning.
