At a temporally worksite located at 736 Spring Haven Drive, in Saint Johns, Florida: On or about July 24, 2023, the employer exposed employees to head injuries, in that protective helmets (hard hats) were not utilized by employees while picking up debris around the home and passing material to the employees at the roof level.

On the roof at a residence, at a temporary worksite located at 736 Spring Haven Drive, in Saint Johns, Florida: On or about July 24, 2023, the employer exposed employees to eye injuries, in that eye protection was not utilized by employees while operating pneumatic nail guns.

29 CFR  1926.1053(b)(1):  Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:   a. At a temporary worksite located at 736 Spring Haven Drive, in Saint Johns, Florida: On or about July 24, 2023, the employer exposed employees to fall hazards, in that the ladder rails did not extend 3 feet above the upper landing surface of the roof at the eave.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. Employees performing roofing work at elevations of six feet or more above the ground are not being protected from fall hazards by a fall protection system: The employer fails to assure effective protection against fall as required by the standard, as demonstrated on July 24, 2023, when six employees performing roofing work on a 8:12 and 5:12 roof pitch of a house located at 736 Spring Haven Drive, in Saint Johns, Florida, were exposed to the fall hazards,  in that employees did not use any fall protection system.    ELO Restoration, Inc. was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1475872, citation number 01, item number 001 and was affirmed as a final order on July 14, 2020, with respect to a workplace located at 424 Blairmore Blvd., Orange Park, FL 32073.  ELO Restoration, Inc. was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1329018, citation number 02, item number 001 and was affirmed as a final order on March 6, 2020, with respect to a workplace located at 397 Bostwick Circle, Saint Johns, FL 32084.  ELO Restoration, Inc. was previously cited for a serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1326756, citation number 01, item number 001 and was affirmed as a final order on March 6, 2020, with respect to a workplace located at 156 Gala Circle, Daytona Beach, FL 32124.  ELO Restoration, Inc. was previously cited for a other-than-serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1318761, citation number 01, item number 001 and was affirmed as a final order on March 6, 2020, with respect to a workplace located at 1116 Nochaway Drive, Saint Augustine, FL 32092.
