29 CFR  1910.334(a)(2)(ii): If there is a defect or evidence of damage that might expose an employee to injury, the defective or damaged item shall be removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made:   On or about August 2, 2023, in the housekeeping area, the power cord to a Sanitaire vacuum cleaner was missing a ground pin.  An employee could become the ground in the system and electricity could travel through the employee which could cause injuries such as severe burns or electrocution.

29 CFR  1910.303(g)(1)(ii): Working space required by this standard may not be used for storage. When normally enclosed live parts are exposed for inspection or servicing, the working space, if in a passageway or general open space, shall be suitably guarded.  On or about August 2, 2023, in the electrical room, the working space in front of the electrical panels was used for the storage of cardboard boxes filled with paper files. Access to the panels in case of emergency may be delayed, exposing employees to burns or smoke inhalation.   Moapa Paiute Travel Plaza was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1559062, citation number 1, item number 2 and was affirmed as a final order on November 24,2021, with respect to a workplace located at Interstate 15, Exit 75, Moapa, NV 89025.

29 CFR  1910.157(c)(1): The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury:  On or about August 2, 2023, employees were not protected from potential fire hazards, in that fire extinguisher were not mounted or identified to inform the employees of the location in case of an emergency on multiple instances.  Instance 1: Fire extinguisher located to the left of entrance to tobacco room from sales floor was not identified.  Instance 2: Fire extinguisher located at the rear of tobacco room was not identified. Instance 3: Fire extinguisher located inside the surveillance room was mounted or identified as it being used to hold the door open.  Instance 4: Fire extinguisher located in the employee breakroom was not identified. Instance 5: Fire extinguisher located in front of business office was not identified.

29 CFR  1910.303(g)(1): Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment:   On or about August 2, 2023, an electrical panel located in the storage area behind the tobacco room was partially blocked by merchandise such as case of water. In the event of an emergency the ability to quickly access to the panel in order to shut down power would be interrupted, thus potentially exposing employees to electrical and fire hazards.
