(a): Excavation - Two employees were in an excavation that was not protected from cave-ins by an adequate protective system. Two employees were inside an excavation with an approximate depth of 10-12 feet with no protective system present on the East, South, and West sides of the excavation, on or about August 28, 2023. There is no abatement certification or documentation required for this item.
