29 CFR  1926.451(g)(3)(i): When vertical lifelines are used, they shall be fastened to a fixed safe point of anchorage, shall be independent of the scaffold, and shall be protected from sharp edges and abrasion. Safe points of anchorage include structural members of buildings, but do not include standpipes, vents, other piping systems, electrical conduit, outrigger beams, or counterweights.   a)At or near 474 Hudson Terrace, Englewood Cliffs - Exterior Frame Scaffold - Stairtower: employee engaged in masonry work was exposed to fall hazards of approximately 42 feet while anchoring his lanyard to a scaffold leg crossbrace anchor pin, on or about 7/29/24.  b)At or near 474 Hudson Terrace, Englewood Cliffs - Exterior Frame Scaffold - Stairtower: employee engaged in masonry work was exposed to fall hazards of approximately 42 feet while anchoring his lanyard to a scaffold crossbrace, on or about 7/29/24.

29 CFR  1926.451(g)(4)(xv): Crossbracing is acceptable in place of a midrail when the crossing point of two braces is between 20 inches (0.5 m) and 30 inches (0.8 m) above the work platform or as a toprail when the crossing point of two braces is between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform. The end points at each upright shall be no more than 48 inches (1.3 m) apart.  a)At or near 474 Hudson Terrace, Englewood Cliffs - Exterior Frame Scaffold - Stairtower: employees engaged in masonry work from a frame scaffold platform without proper railing were exposed to fall hazards of approximately 42 feet, on or about 7/29/24.

29 CFR  1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.   a)At or near 474 Hudson Terrace, Englewood Cliffs - Exterior Frame Scaffold - Stairtower: employees engaged in masonry work around  a frame scaffold platform without any toe boards were exposed to struck by hazards from tools and materials that could fall approximately 42  feet, on or about 7/29/24.

29 CFR  1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.   a)At or near 474 Hudson Terrace, Englewood Cliffs - Frame Scaffold - Stair tower: employees engaged in masonry work were exposed to fall hazards of approximately 42 feet while climbing the "Walk Through" scaffold section on the top portion of the tower, on or about 7/29/24.  b)At or near 474 Hudson Terrace, Englewood Cliffs - Frame Scaffold - Elevator tower: employees engaged in masonry work were exposed to fall hazards of approximately 30 feet while climbing the "Walk Through" scaffold section on the top portion of the tower, on or about 7/29/24.  FAST Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.451(e)(1), which was contained in OSHA inspection number 1677741, citation number 01, item number 01 and was affirmed as a final order on 6/17/23, with respect to a workplace located at 225 Chosin Few Way, Bayonne NJ
