29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) 530 Pine Street, Lykens, PA Roof - Employees performed residential roofing activities on a two-story residential roof,  approximately 18 feet above grade with an 6/12 roof pitch, without utilizing any means of fall protection, on or about December 14, 2023.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    (a) 530 Pine Street, Lykens, PA, Ridgeline Roof, - Employees were performing residential roofing operations on a two story residential roof,  the portable ladder did not extend 3 feet above the upper landing, on or about December 14, 2023.  (b) 530 Pine Street, Lykens, PA, Porch Roof, - Employees were performing residential roofing operations on a two story residential roof,  the portable ladder did not extend 3 feet above the upper landing, on or about December 14, 2023.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1926.95(a):Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided:  (a) 530 Pine Street, Lykens, PA, Roof, - Employees did not don safety glasses while using pneumatic nail guns to attach shingles to the roof of a two story residential  building and were exposed to flying debris and ricochet nails, on or about  December  14, 2023.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
