29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: a- 1625 White Cedar Lane, Williamstown NJ: On or about 04/23/24 a worker was exposed to fall hazards, in excess of 17 feet 6 inches to the ground surface below, when working/walking from a stack of three unsecured 4 x 8 sheets of OSB, resting on the elevated forks of an all terrain fork truck. This piece of equipment was not specifically designed by the manufacturer to support a scaffold platform.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: a- 1625 White Cedar Lane, Williamstown NJ: On or about 04/23/24 a company worker was exposed to fall hazards, in excess of 17 feet 6 inches to the ground surface below, when working/walking from 3 - 4 foot by 8 foot sheets of OSB (one stacked on top of the other) that were resting, unsecured on the elevated forks of a Pettibone Traverse Lift All-Terrain fork truck while they moved horizontally. At the time of the exposure, the worker was not using fall protection.
