29 CFR 1910.23(c)(3): The employer did not ensure that ladders were not loaded beyond the maximum intended load:  Pacific Views Lodge, Maintenance Shop: On and/or before January 30, 2024, HUSKY 6-foot A-frame ladder was used to access heights over 6 feet. The A-frame ladder used did not have any manufacture data listed for safety requirements nor load limits for employees to identify during inspection and use of the ladder. Employees were exposed to the potential of the ladder overloading, mechanical failure and/or fall hazard(s).

29 CFR  1910.151(c): The employer did not provide appropriate emergency facilities for quick drenching or flushing of the eyes and body where employees may be exposed to injurious corrosive materials within the work area.  Pacific Views Lodge, Kitchen: On and/or before January 30, 2024, the employer did not insure that a plumbed eyewash station was available for their employees to use in case of emergencies where rinsing of the eyes for 15 minutes or more would be necessary. This condition potentially exposed employees to moderate to severe eye irritation and chemical burns.

This condition interferes with access and immediate shutdown of a circuit in the panel, particularly in the event of an emergency.
