29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. On or about February 8, 2024, at an establishment located at 2660 West Mason Street in Green Bay, Wisconsin, the employer did not provide machine guarding to protect operators and other employees in the machine areas from hazards during the cycle of operation. Employees were exposed to pinch points and rotating parts on the jump ringing machines in the assembly department.

29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. At an establishment located at 2660 West Mason Street in Green Bay, Wisconsin, employees of The Medalcraft Mint, Inc. were exposed to electrical and fire hazards utilizing listed equipment not in accordance with the instructions for a Relocatable Power Tap (RPT) that was not designed to be installed in an industrial setting in the following settings: a) On or about February 8, 2024, in the cleaning room, an employee was utilizing the water cleaning tanks with two water filters plugged into the RPT that was not designed to be in a wet location. b) On or about February 8, 2024, in the enameling department, employees were utilizing RPT's at their workstations to provide power to various equipment in the production process.
