29 CFR  1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  A.) Corry Fabrication, LLC, on or about 2/22/24 and times prior thereto: Specific Lockout Tagout procedures were not developed for equipment including but limited to oven, crane, and laser.      ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1910.147(e)(3):Specific procedures and training that were developed, documented and incorporated into the employer's energy control program for removal of a lockout or tagout device under the direction of the employer when the authorized employee  A.) Corry Fabrication, LLC, on or about 2/22/24 and times prior thereto: employees periodically conducted service, maintenance, and repairs on equipment.  The energy control procedure developed for use during removal of a lockout device by someone other than the authorized employee who applied it situations, were not specific and/or require specific procedural steps prior to and after the removal of the lock.  Equipment includes but is not limited to  cranes, lasers, and hydraulic equipment.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement

29 CFR  1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures:   A.) Corry Fabrication, LLC, on or about 2/22/24 and times prior thereto: The employer periodically used outside contractors for servicing maintenance and repairs to equipment in the facility including but not limited to, the elevator and the burn off oven.  specific procedures were not developed and implemented for outside contractors performing lockout.     ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). The employer did not conduct Lockout Tagout/Hazardous Energy Control training with Authorized employees.   B.) Corry Fabrication, LLC, on or about 2/22/24 and times prior thereto: Employees work around equipment that requires periodic service and maintenance. The employer did not conduct Lockout Tagout/Hazardous Energy Control training with Affected employees. Corry Fabrication, LLC, on or about 2/22/24 and times prior thereto: Lockout devices were not distinguished from other devices in the workplace.  Other devices include but are not limited to locks to take equipment out of service. Red locks were used as both lockout devices and locks to take equipment out of service.       ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
