OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by or caught-in hazards while operating equipment that have unprotected foot pedals:  A.) Corry Fabrication, LLC, On or about 3/13/25 and times prior thereto: Employees operated the Cincinnati 9 B12 Mechanical Press Brake that uses a foot pedal for actuation.  The foot pedal was not protected from actuation from falling or moving objects, or by inadvertent stepping on the foot pedal exposing employees to the hazards of getting hands and fingers struck-by or caught-in the press brake point of operation.   B.) Corry Fabrication, LLC, On or about 3/13/25 and times prior thereto: Employees operated the Cincinnati  B13 Mechanical Press Brake that uses a foot pedal for actuation.  The foot pedal was not protected from actuation from falling or moving objects, or by inadvertent stepping on the foot pedal exposing employees to the hazards of getting hands and fingers struck-by or caught-in the press brake point of operation.   ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

Corry Fabrication, LLC, On or about 3/13/25 and times prior thereto: Employees did not lockout prior to servicing operations on equipment including but not limited to Shears.  Employees are periodically required to change blades in the Cincinnati S1. I.)	Corry Fabrication, LLC, On or about 3/13/25 and times prior thereto: Employees did not lockout prior to servicing operations on equipment including but not limited to Band Saws. Employees are periodically required to change blades in the 55 DoALL. Tasks include but are not limited to changing dies, blades, and tools. Corry Fabrication, LLC, On or about 3/13/25 and times prior thereto: The Cincinnati B12 Press Brake was not adequately guarded to prevent employees from contacting the point of operation in that there were no safeguarding devices used in conjunction with the foot pedal.    B.) Corry Fabrication, LLC, On or about 3/13/25 and times prior thereto: The Cincinnati B13 Press Brake was not adequately guarded to prevent employees from contacting the point of operation in that there were no safeguarding devices used in conjunction with the foot pedal.    C.) Corry Fabrication, LLC, On or about 3/13/25 and times prior thereto: The Pierce All P4 Punch Press was not adequately guarded to prevent employees from contacting the point of operation in that there were no safeguarding devices used in conjunction with the foot pedal.  D.) Corry Fabrication, LLC, On or about 3/13/25 and times prior thereto: The Cincinnati S1 was not adequately guarded to prevent employees from contacting the point of operation in that a section of the guard was broken off/missing. 29 CFR  1910.217(b)(7)(iii): A means of selecting Off, "Inch," Single Stroke, and Continuous (when the continuous function is furnished) was not supplied with the clutch/brake control to select type of operation of the press. Fixing of selection was not by means capable of supervision by the employer.    A.)	Corry Fabrication, LLC, On or about 3/13/25 and times prior thereto: Press Keys were left in machines, which allowed employees to change the mode of the machine. Keys were not designated for authorized employees/supervisors.  All employees could change the mode of the press, presses include but is not limited to, Bliss C-60.
