29 CFR  1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:   A.) Corry Fabrication, LLC, On or about 3/13/25 and times prior thereto: Knockout receptacle boxes were being used to power equipment including but not limited to portable welders.  The knockout box receptacle was located on the floor. Knockout Receptacle boxes are not listed and labeled for use on the floor and are required to be surface mounted.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors were not replaced:   A.) Corry Fabrication, LLC, On or about 3/13/25 and times prior thereto: Employees are using damaged cables for welding operations associated with equipment including but not limited to, Lincoln Electric 60-179 in the first-floor chip blaster area exposing employees to potential electrical hazards.  Insulation was stripped back from the wiring exposing the copper interior of the ground wire.   B.) Corry Fabrication, LLC, On or about 3/13/25 and times prior thereto: Employees are using damaged cables for welding operations associated with equipment including but not limited to, Lincoln Electric 60-960 in the first-floor welding cell exposing employees to potential electrical hazards.  Cords were wrapped in black electrical tape.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Corry Fabrication, LLC, On or about 3/13/25 and times prior thereto: extension cords were observed with electrical tape repairs. Damaged Electrical cords are used to power equipment including but not limited to, radios, and power tools  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). 29 CFR  1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form.   a.) Corry Fabrication, LLC, located at 21 Maple Ave, Corry, PA 16407, on or about 3/13/25, and times prior thereto: The employer did not evaluate respiratory hazards in areas where respirators are required to be worn.  Respirators were required in areas including but not limited to, sandblasting, powder coating, welding, lasers.   ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
