29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse, (a) On or about March 5, 2024, racking systems used for storage of parts and materials, created a crushing and struck-by hazard, in that the system was damaged in excess of the allowable limit of 0.5 inches of deflection within a 10 foot span, per ANSI MH16.1: 2021 'Specification for the Design, Testing and Utilization of Industrial Steel Storage Racks.' Specifically, but not limited to the following damaged rack areas were identified: The Green Line Rack: The horizontal beam was damaged with a deflection greater than 6 inches, creating a collapse and struck by hazard.

The cover plate was secured in place with tape.
