29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not stored in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: (a) On or about August 29, 2024, the employer did not follow the Compressed Gas Association Pamphlet P-1-1965, in-that empty propane tanks stored under the Mug Hog platform were not protected / secured in place from being tipped over, or struck by other equipment or materials, creating a potential fire hazard and struck by hazard.

29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled. (a) On or about August 29, 2024, a 55 gallon plastic drum full of water slurry was transported on top of the forks of a Mitsubishi Forklift without securing the drum in place to prevent displacement of the drum while being transported, exposing employees to a struck by hazard. (b) On or about August 29, 2024, employees handling a 55 gallon plastic drum full of water slurry elevated on the forks of a Mitsubishi Forklift were exposed to a crushing, pinch point and amputation hazard while dumping the water slurry into a hopper.
